Steps (Clinical Judgment Model):
- Recognize cues – Gather info (subjective = what patient/family says; objective = what you observe).
- Analyze cues & prioritize hypotheses – Decide what the problems are and which are most urgent.
- Generate solutions – Create realistic, measurable, patient-centered goals (shared with patient + health team).
- Take action – Do nursing interventions: teach, monitor, guide, consider side effects/diet/self-care.
- Evaluate outcomes – Did it work? Were goals met? Document results.
