External structures normal: eyebrows and eyelashes evenly distributed, eyelids symmetrical, no edema or lesions.
Sclera white, conjunctiva clear, iris round with even color. Lens transparent.
Pupils equal, round, reactive to light and accommodation (PERRLA).
Extraocular movements intact in six cardinal fields, no nystagmus. Cover–uncover test normal.
Eyelids and surrounding tissues soft, non-tender, no nodules.
Visual acuity 20/20 bilaterally with Snellen chart.
Near vision intact with Rosenbaum card.

👃 Nose & Sinus – Normal Report

Nose midline, septum midline without deviation, mucosa pink and moist, no drainage or lesions.
Nares patent bilaterally with equal airflow.
Client correctly identifies familiar scent; CN I (olfactory) intact.
Frontal and maxillary sinuses non-tender on palpation.

Lips, oral mucosa, gums, tongue, tonsils, and palate pink, moist, without lesions. Teeth intact, no swelling or drainage.
Tongue midline, uvula rises midline with phonation, soft palate elevates symmetrically.
Movements of tongue and palate smooth, coordinated, and symmetrical. CN IX, X, and XII intact.
