🩺 Nursing Process

  1. Assessment – Collect data (subjective + objective).
  2. Analysis – Interpret data, identify patient problems.
  3. Planning – Set goals and outcomes for care.
  4. Nursing Interventions – Carry out the plan.
  5. Evaluation – Check if goals were met and adjust if needed.

👉 It’s the outcome of critical thinking + decision making in nursing.

Steps (Clinical Judgment Model):

  1. Recognize cues – Gather info (subjective = what patient/family says; objective = what you observe).
  2. Analyze cues & prioritize hypotheses – Decide what the problems are and which are most urgent.
  3. Generate solutions – Create realistic, measurable, patient-centered goals (shared with patient + health team).
  4. Take action – Do nursing interventions: teach, monitor, guide, consider side effects/diet/self-care.
  5. Evaluate outcomes – Did it work? Were goals met? Document results.
